Snomed CT - Code Request Form

If you are a Vision customer in Scotland, please contact your local facilitation team to request any SNOMED CT Terms.

If you need to be able to use Vision 3 search and reports for a SNOMED CT code that we have not currently mapped to Read, please let us know the SNOMED description (the text associated with the numeric code) and also the SNOMED CT ConceptID if possible.
